If you are user Example, and you have just checked an article for COI by user John Smith who has declared his connection at the diff with revision ID 442212827, use this:

Use this template when dealing with a general conflict of interest, including financial conflict of interest that does not involve being paid to edit Wikipedia. It can be used whether there is an actual (confirmed) or an apparent COI, but if the COI is unconfirmed do not add it without justifiably strong suspicion.
The template adds articles to Category:Articles with connected contributors.

Parameters

- Text parameter. Most recent date that a neutral editor checked the edits of all known connected contributors for neutrality as well as the name of that editor. For example:

- Text parameter. User name of the COI editor. For example:

- Note that the user name is automatically linked complete with talk page and contributions (i.e. the {{user}} template is automatically applied for you)

- Text parameters. Alternative names for the COI editor. Useful in cases of Wikipedia:Sockpuppetry. Otherwise works like

- Yes/no parameter (optional). If the COI editor has edited the present article, enter "yes".
|declared=
- Provide the diff number in which the COI editor has declared their connection/financial interest in Wikipedia. Can also be used as a yes/no parameter if there is no diff. Optional.
|otherlinks=
- Text parameter (optional). Provide additional information about the declaration. Formerly commonly used to provide the diff link.
|banned=
- Yes/no parameter (optional). If the COI editor user has been formally banned from editing the present article, enter "yes". This adds a note to that user name, informing readers that if they spot contributions of that user that weren't caught and reverted, they may do so. Please do not use this parameter if you are not sure about the COI editor's status.
|small=
- Yes/no parameter (optional). If you need to make the box small and right floating, enter "yes".
Multi-listing

This template is capable of listing up to 10 editors. Assign a number between 1 and 10 to each editor listed, and substitute that value for X in each of the parameters listed below. Parameters otherwise work as described for single-listing above.

- the user name of the COI editor. Note there is no capacity to list alternative names (per the

parameters above) in a multi-listing. - UX-EH
- Edited here: whether or not the COI editor has edited the present article
- UX-declared
- COI declared: whether the COI user has declared their connection; can take the diff if yes to create a link
- UX-otherlinks
- Other links: a link to the diff of the editor's COI declaration
- UX-banned
- Banned: whether the user has been formally banned from editing the present article
