Topic relevance: The paper is on portocaval shunts. Because the shunts are no longer used as frequently due to advancements in IR and the TIPS procedure, you could expand upon the procedure's history if this information is available.

Neutrality: The author does not appear to be persuaded by any particular point of view. The overall tone is neutral.

Citations: I was able to access all of the citations in the references section. Notably, The Japanese Journal of Surgery is from 1974. You can get further information from this journal regarding the history of this procedure.

Clarity: The first sentence of the lead could be divided into multiple sentences to make it more straightforward to the reader. The first sentence needs to be clarified about the purpose of the portal vein. It states that it supplies blood to the liver, but from where and is it oxygenated?

Following PBC in the lead, “Budd” is mentioned again.

Contrasting portocaval shunts and TIPS would be helpful to the reader.

The image under “types of shunts” is confusing at first glance. Is there potentially an image with less going on to make it simpler for the reader? Also, why choose one shunt over the other?

Under Outcomes, clarify where this bleeding comes from and why, “Studies have shown that surgical shunting is highly effective in controlling the bleeding.”
