Cleanup

In rough order of priority:

Alphabetising scheme needs standardising and applying.
Sections may need re-writing to better continuous prose.
"Appears in" introductions could be changed to the standard wiki "Heading is a" style.
<ref> tags could suffice in some cases for which episode they appear in.
