Gmina Małkinia Górna is a rural gmina (administrative district) in Ostrów Mazowiecka County, Masovian Voivodeship, in east-central Poland. Its seat is the village of Małkinia Górna, which lies approximately 16 kilometres (10 mi) south-east of Ostrów Mazowiecka and 90 km (56 mi) north-east of Warsaw.

The gmina covers an area of 134.08 square kilometres (51.8 sq mi), and as of 2006 its total population is 12,224 (12,048 in 2013).
