In most cases these are pictures that are linked from an article, but with text links that users must click, rather than the image (or a thumbnail) being directly displayed as part of the article. This is done for images that are relevant to an article but need not always directly display with that article (because they might otherwise overload the article, because the image contains possibly disturbing or non-work-safe content, or for other reasons). The purpose of this template is to provide a verifiable declaration that an image is in fact used in some way in relation to a Wikipedia article, even if automated methods for finding undisplayed "orphan" images would consider it to be an unused image. Note that the word "orphan" derives from the terminology used to describe tree data structures in computer science
(Some of these files are used under a claim of "fair use" - careful consideration should be given to whether their current usage can be considered to contribute significantly to the article they are linked from, and otherwise qualify as a valid "fair use" claim.)
Also, keep in mind that Wikipedia articles might be reproduced in printed form, and that media which can only be viewed by following a hyperlink might be "lost" to downstream readers.
